GENERAL OBJECTIVE

To increase the percentage of individuals receiving COVID-19 1st booster dose among the fully-vaccinated residents of Barangay Lanzones from 29% to 50% by the end of 2023.

To identify fully-vaccinated individuals who haven’t received the booster shot of COVID-19 vaccine.

SPECIFIC OBJECTIVE 1

Our Proposed Solution

Coronavirus disease is highly transmissible and can pose a great threat to an individual's health, however through vaccination, the vulnerable population can be protected from its fatal complications. COVID-19 vaccine boosters can further enhance or restore protection that have waned over time after receiving the primary series of vaccination, especially with further mutation. Vaccine hesitancy is a big hindrance which can be addressed through a series of health education and dialogues with the residents. Strategies employed in this community health plan includes:

Strengthening of the partnership with the stakeholders
Intensified IEC campaign & FGD
Synchronized “Catch & Change: Covid-19 Booster” and IEC.

Sustainability

The creation of FGD core members, with the permission and approval of the barangay officials, that will be composed of the organic members of the barangay such as the BHWs, Purok Leaders, and other willing volunteers, and by providing them technical support will help ensure the sustainability of this initiative even without the presence of the medical students. Continuous mentoring and monitoring will be implemented through the facilitation of the health district staff. Program Implementation Review on the “Catch & Change: Covid-19 Booster” will also be performed at the end of the implementation phase, this will give opportunity to the top 3 core members with the highest number of converts to be recognized.
