This community health plan was conducted as a response to the call for bringing health to and for the people. The purpose of this Community Health Plan specifically focused to (a) Describe the demographic profile, educational attainment, living and environmental condition, socio-economic profile, habits and practices, and health status of the community; (b) Identify and describe the health problems present in the barangay; (c) Determine solutions for the identified health problems with the help of the community; (d) Implement the community health plans including the proposed solutions to the identified health problems, and lastly; (e) Evaluate effectiveness of community health plans on the identified health problems of the people of Barangay Lanzones, Zamboanga City.

How can we help?

The framework of the community health plan adapted and inspired by UNICEF’s framework on Community Health System for Survival, Healthy Growth and Development. This facilitated the group’s visualization of various factors within the community. On the left side of the framework, the group identified community networks as a crucial component, encompassing supportive pillars such as the Local Government Unit and social community groups. These entities often serve as immediate support for primary caretakers, alongside with the sectors of the local governance which then forms the backdrop within which the community operates. On the other hand, the Barangay Health Care Workers play a pivotal role as the primary link between the caretakers and the initial level of care – the primary health care facility, extending its connections to the broader service delivery chain as illustrated on the right side of the diagram. To strengthen operational community networks and enhance health service delivery, the group of medical students, together with the community residents and barangay health workers, intervene by addressing identified community health issues through various strategies and activities in the need for empowering the community on the targeted health issues.
