GENERAL OBJECTIVE
To increase the number of controlled hypertension among 20 to 59 years old from 0% to 30% with a BP goal of less than 140/90mmHg and among 60 years old and above from 60% to 80% with a BP goal of less than 150/90 mmHg in Barangay Lanzones by the end of 2024.
Our Proposed Solution
The Department of Health (DOH) in the Philippines classifies Hypertension as one of the major non-communicable diseases (NCD), as it is the most important risk factor for any cardiovascular and kidney diseases, as well as stroke and even death if not detected early and managed appropriately. Proper management is clearly important to each individual with Hypertension, where health education about the adherence to pharmacologic and non-pharmacologic interventions will effectively improve their blood pressure (BP) and set of lifestyles.
Sustainability
The identified high-risk and clinically diagnosed hypertensive individuals are prospectively the candidates for the program initiated for the community, such as screening, health education, referral system, monitoring scheme, and “Aksyon kontra AltaPresyon” modification program. With these interventions initiated, this will provide a sustainable approach in the community care setting for the long-run in managing risk factors and complications.
